@NKartha Thanks to your team for this release. The Forcible Entry and Detainer intake is a good start. I would request some changes as follows:

  1. Add section for “Eviction Reason”. 1st item is dropdown box “Eviction Reason” with options: non-payment of rent; lease violation; non-renewal/holdover; squatter. 2nd item is NTQ Question already present in form. 3rd item is attachment of served NTQ based on conditional using 1st and 2nd item - If non-payment then 5-day notice; If lease violation then 10-day notice; if non-renewal/holdover then notice of non-renewal; squatter does not require NTQ but one could be served in case they claim sublessor status.
  2. Add attachments for files: written lease (if selected) OR missing lease affidavit if written lease selected but it is missing; tenant ledger.
  3. Add section regarding relationship of Plaintiff to the property. For example, is plaintiff the owner, corporate officer, property manager, etc. If corporate officer or agent or property manager then add a document attachment for documents demonstrating legal relationship to property.
  4. Add section to specify who will be the witness to prove up the complaint - lease, ledger, etc.
  5. Add a question about whether the tenant’s lease term began prior to Plaintiff’s ownership or management of the real estate; if so, add question whether Plaintiff is seeking rents from prior to Plaintiff’s relationship to property; if so, add attachment for assignment of rent to Plaintiff from prior interest holder.
