Disclaimer on Intake Form

If you would like to add a disclaimer to appear as the client completes the form, you can achieve this by using the 'Add Paragraph' option in the form builder to include your statements.
 

If you would like to add a disclaimer to appear as a confirmation message when the form is completed. You can customized your confirmation message via the settings section in Smokeball (see Confirmation Message). This message will appear for all forms completed by your clients. 

 

Alternatively, you can set a specific message for each intake form by accessing the Settings menu within the Intake Form builder.
